ACL National Chronic Disease Self-Management Education Resource Center

Through this funding opportunity, the Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award one cooperative agreement, with two goals:
Goal 1:
Provide technical assistance, education, and resources for the national CDSME network of partners to increase

credit:


the number of older adults and adults with disabilities who participate in evidence-based CDSME and self-management support programs.

Goal 2:
Support the development and enhancement of integrated, sustainable service networks within states, regions, and tribes that provide evidence-based CDSME and self-management support programs, as well as related services that address the social and behavioral determinants of health among older adults and adults with disabilities.
